Case #1:
Guy comes in with a rash progressive for a year. RPR negative (which is rare here, since malaria can make it positive too). Not better with doxy, erythro, griseo, ceftriaxone, mebendazole, praziquantal, ivermectin and diethylcarbamazepine. Been here for over a week. Any ideas?
Case #2
Twelve-day-old comes in with fever, abscess of the bicep and buttock and rash below. Has malaria. Fevers gone after ampicillin and gentamicin. Rash persists. Any ideas?

Streptomycin...that will cover plague, tularemia and tb. Just a thought. Not sure if you can get the streptomycin..if not let me know...just a though. This for case #1

ReplyDeletethe ist case looks like malignancy, his umbilicus is retracted/distorted as in sister mary joseph nodule, ie bowel cancer... he also has raised groin nodes and what looks like suppurating axillary/pectoral lymph nodes.... plus no improvement with antibiotics. jo. good luck ! btw once saw a baby with lupus had similar rash in case 2.
